Break DGU package form into sections

• use javascript to selectively hide/show parts of the form
• on the creation form, there are "Next" and "Back" buttons. And the workflow is controlled to only enable tabs when previous section has been completed.
• there's no validation between steps at this stage. It's still a "big save button at the end".
